How to SIGN UP for Better at Home

We have many ways you can sign up for the program:

Download our Intake Form HERE.

    1. Print it out and send photos of the completed pages to betterathomecortes@gmail.com
    2. Send Ester our program coordinator an email: betterathomecortes@gmail.com and she can do the intake over the phone.
    3. Call Ester our program coordinator at 250.204.2116 (local) and leave a message and she will call you back to sign you up over the phone or mail the forms to you.
    4. Find a copy of the forms at the Health Centre or at any Post Office on Cortes and send your completed forms to:
